Preparing for Your Visit

We want your experience with us to feel welcoming, seamless, and informative from the moment you arrive. Preparing a few important items ahead of time can help us make the most of your visit and ensure you receive personalized, efficient care.

Before your appointment, please bring or be prepared with:

  • Your medical history, including past diagnoses, surgeries, or major health concerns

  • A list of current prescription medications and dosage instructions

  • Any over-the-counter medications or supplements you regularly take (bringing the actual bottles can be especially helpful)

  • Your insurance card and information

We also encourage you to contact our office prior to your visit to confirm that your insurance plan is accepted.

Referral & Insurance Information

Many HMO insurance plans require a referral from your primary care physician before seeing a specialist. Without an approved referral, your appointment may need to be rescheduled.

Because some referrals also require authorization from your insurance company, we ask that you allow at least three business days for referral processing.

Please note:

  • We are unable to backdate referrals

  • Referral processing times may be longer if insurance authorization is required

  • Referrals are not faxed; patients are responsible for delivering referrals to the specialist’s office

  • Referral pickup hours are Monday–Friday from 8:30 a.m. to 4:00 p.m., unless other arrangements have been made

For procedures or surgeries, the specialist’s office must request the referral directly. Once received, we will send the referral to the specialist on your behalf.
